ADDICTION MEDICINE
Our clinic offers comprehensive treatment for patients living with substance use disorders. Patients with substance use disorders are initially seen in our intake addiction clinic, which takes place once each week. This clinic offers all medications for opioid dependence (sublingual and injectable buprenorphine, naltrexone, opioid tapering consultations). The addiction clinic also practices an integrated mental health model where patients are linked into behavioral health as they receive their comprehensive medical treatment. Our intake clinic recruits new patients via strong partnerships with community organizations and facilities that treat patients with substance use disorders. The addiction intake clinic is led by Dr. John Hayes, DO; a double boarded family and addiction medicine faculty physician with extensive experience in the treatment of substance use disorders. Patients are seen weekly in our addiction specialty clinic until their addiction stabilizes. Patients are then dispersed into faculty and resident panels and seen any day of the week. All of our faculty have buprenorphine training and can staff addiction medicine cases alongside any other medical issues. We offer comprehensive addiction, mental health, medical, birth control, and prenatal care in our clinic. We also offer treatment to patients with HIV and Hepatitis C.

Our clinic participates in behavioral health collaborative case management. High risk patients with comorbid psychiatric and substance use disorders are regularly discussed in weekly case management rounds led by our RN Case manager. Our multidisciplinary case management team consists of primary care physicians, addiction medicine fellows and attendings, a RN case manager, a psychiatrist, social workers, and peer support specialists. In rounds, we discuss at-risk individuals and collectively brainstorm how each of us can collaborate to help our patients.

The Forest Home Health Center’s lobby features a harm reduction vending machine supplied by the Milwaukee County Public Health Department! Anyone in our community is welcome to access FREE naloxone, gun locks, drug disposal bags, and locked medication storage bags.

We provide comprehensive Family Medicine care for our patients including preventive care, chronic condition management, obstetrics, pediatrics, and geriatric care. Our clinic is also able to provide full spectrum primary care procedures including musculoskeletal injections, contraception management (including IUD/Nexplanon), colposcopy, and dermatology procedures. Our faculty physicians have unique expertise in maternal child health, addiction medicine, sports medicine, osteopathic manipulation, urgent care, and behavioral health. We provide services in both Spanish and English and can use translators for other languages as well. We are always welcoming new patients and families to join our health center and community! Our behavioral health team has expertise in psychodynamic therapy, family therapy, and helping couples improve communication.

MATERNITY CARE
Many faculty physicians within our clinic are trained in high-risk obstetric care and are able to follow our patients from before pregnancy, throughout prenatal care including performing vaginal deliveries and postpartum care. We then also are able to care for the newborns after delivery and throughout their childhood optimizing continuity of care.

MUSCULOSKELETAL HEALTH
Our clinic is able to provide comprehensive musculoskeletal care for patients including injections, treatment of common injuries, and many of our faculty are trained in osteopathic manipulation therapy (OMT) to assist patients in treatment for neck and back pain among other common injuries.
