The continuity clinic home of the CARE fellowship is the Froedtert and Medical College of Wisconsin’s Forest Home Health Center, located in the heart of the 53204 zip code that is most heavily affected by the drug overdose crisis in Milwaukee.
The community is approximately 70% Hispanic/Latino according to Milwaukee census data. Approximately 30% of these families are living below the poverty line, and many prefer to converse in Spanish language. The clinic is staffed by bilingual clinicians and support staff, offers social work and other social determinant of health support, and engages deeply with community partners in order to provide the best and most appropriate care possible. The clinic is also the home site of the MCW/Froedtert South Side Family Medicine Residency Program, offering opportunities to teach and learn, and impact the education of resident physicians and medical students. The neighborhood the clinic serves is a designated health professions shortage area with a HPSA score of 12 and is a designated medically underserved area by HRSA.
Fellows will also provide care at Outreach Community Health Center, located on the north side of Milwaukee. This clinic is the home site of the MCW/North Side Milwaukee Health Centers Family Medicine Residency Program. This site is also located in an area of high healthcare need, but in a community that is demographically majority Black/African-American. At this clinic site, fellows will teach resident physicians and staff designated substance use clinic sessions.
